Starting A California Workers' Compensation Claim
In order to start a workers compensation claim in California, you must first give your employer "notice" of the injury, or tell them that you are injured. Once you notify your employer, your employer is supposed to provide you with a worker's compensation injury claim form. A copy of the California workers compensation claim form (Form DWC-1) is provided in the "Forms" section of this book.
Whether your employer gives you a claim form or you obtain one for yourself, you are supposed to fill it out and give it to your employer. There is a section that your employer is supposed to fill out. Then, your employer is supposed to give a completed copy back to you. By submitting this claim form to your employer, you are formally initiating your worker's compensation claim.
